- Strong 9M26 growth, but the outlook is becoming more challenging: Revenue rose 35% to USD 131.8 million, with growth in both the US and Rest of World. However, FY26 revenue is now expected to fall below the previous USD 175–185 million guidance, while management expects continued pressure on the US business in FY27.
- US competition is intensifying: Zydus and Provepharm have both entered the US market, while UltraGreen.ai has relaunched its own generic. The group retains an established market position, but faces greater competition than in our previous assessment.
- The generic relaunch helps defend market share: Offering a lower-priced alternative to IC-GREEN may help retain customers who might otherwise switch to rival generics, although pricing pressure remains a concern.
- New growth avenues offer longer-term potential: Imaging systems and software are expanding into new markets and applications, including outpatient care and wound management, but are not yet expected to contribute meaningfully to revenue.
- Downgrade to TRIM: We have lowered our EPS forecasts and revised our target price to USD 0.66. The implied 16.8% upside is insufficient to compensate for the heightened risks to earnings and market share as competition intensifies.
Company Update
UltraGreen.ai (SGX: ULG)
TRIM: USD 0.66 (16.8%)
UltraGreen.ai is entering a new phase of competition just as demand for fluorescence-guided surgery continues to expand. After years of leading the US indocyanine green (ICG) market, the group now faces two new entrants seeking a share of a market it has long dominated. Its 35% revenue growth in 9M26 shows that the business remains on a strong footing, but the key challenge is whether it can defend its position as competition reshapes the market.
Financial Updates
9M26: Growth remained strong, but the impact of competition is now reflected in guidance
Excluding the discontinued UltraLinQ business, UltraGreen.ai’s revenue for the nine months ended 30 September 2026 increased 35% year-on-year to USD 131.8 million. US revenue rose 35% to USD 101.3 million, while Rest of World (RoW) revenue increased 36% to USD 30.5 million.

On a quarterly basis, however, revenue has levelled off. 3Q26 revenue of USD 44.6 million was around 1% lower than the USD 45.2 million recorded in 2Q26. US revenue edged up 4% quarter-on-quarter to USD 35.2 million, while RoW revenue declined 18% to USD 9.4 million from USD 11.4 million. Management attributed the quarter-on-quarter variation mainly to the timing of orders.
Figure 1: 3Q26 revenue levelled off quarter-on-quarter as a dip in RoW offset US growth

The more important development was the revision to full-year guidance. Management now expects FY26 revenue to fall below its previous guidance of USD 175–185 million, citing increasing pricing pressure and changes in customer purchasing patterns following the entry of additional ICG products. The low end of the previous guidance range implied 4Q26 revenue of around USD 43 million, broadly in line with the quarterly revenue run-rate in 2Q26 and 3Q26. The revised guidance now implies a weaker 4Q26 than previously anticipated, likely reflecting the initial impact of generic competition. Looking further ahead, management expects the US business to remain under pressure in FY27, as the full-year effects of competition weigh on both revenue and profitability, while continued growth is expected outside the US.
Capital returns provide some downside support, but do not replace earnings visibility
The Board has approved a capital allocation framework that prioritises reinvestment in the business, followed by sustainable dividends, strategic acquisitions and share buybacks. Under the framework, UltraGreen.ai targets an annual dividend payout of at least 30% of net profit excluding exceptional items, with distributions made semi-annually. The group has also announced a multi-year share buyback programme of up to USD 30 million, which management indicated would run for more than a year. The balance sheet remains strong, with USD 214 million in cash and short-term investments and no debt as at the end of September.
These measures provide some support for the shares, although their sustainability remains tied to the group’s earnings trajectory. Dividends are linked to net profit and will therefore be affected by any deterioration in earnings. Similarly, while the share buyback programme provides an additional avenue for capital returns, buybacks rank last among the Board’s capital allocation priorities and remain subject to market conditions and the group’s capital needs. With earnings visibility weakening amid intensifying competition, the group’s ability to sustain these capital returns over time becomes less certain.
Outlook
The entry of two competitors marks a shift in the US ICG market, where UltraGreen.ai has historically held an estimated 80% share of volumes. Their entry reflects the growth potential of fluorescence-guided surgery, but also challenges the market position that has underpinned the group’s earnings and valuation.
The underlying market opportunity remains intact. The global ICG market was valued at USD 180 million in 2025 and is projected to reach USD 380 million by 2031, representing a 13.26% CAGR. Clinical evidence is also strengthening the case for wider adoption. A September 2026 meta-analysis of randomised trials found that ICG fluorescence angiography reduced the risk of anastomotic leaks in colorectal surgery, supporting its broader use in surgical procedures.
For UltraGreen.ai, the key question is whether it can defend its leading position as the market expands and competition intensifies. Its established ICG franchise and the wider adoption of fluorescence-guided surgery continue to support demand, but the entry of two competitors increases the risk of pricing pressure and market-share erosion.
Competition is now live, and UltraGreen.ai’s own generic defends market share rather than price
Zydus and Provepharm have both entered the US market, while UltraGreen.ai has relaunched its own generic ICG alongside its branded IC-GREEN.
Provepharm’s entry differs from Zydus’s. Its product, Zyogreen, was launched in the US on 22 September through Sagent, which describes it as the first and only ready-to-use ICG injection in the US. Supplied as a sterile solution, Zyogreen eliminates the reconstitution step required for conventional ICG powder and is marketed through Sagent’s nationwide distribution network.
Zyogreen therefore competes on operating-room workflow as well as price. UltraGreen.ai’s ICG is supplied as a lyophilised (freeze-dried) powder, and the group has not disclosed a comparable ready-to-use product. Provepharm has also indicated plans to expand further across Europe and other international markets from 2027, following its UK launch in July. This will introduce competition into markets that have been an important source of growth for UltraGreen.ai.
UltraGreen.ai’s own generic provides a lower-priced alternative to IC-GREEN, helping retain customers who might otherwise switch to competing generics. The strategy is therefore aimed at defending market share rather than preserving pricing power. Although IC-GREEN may continue to command a premium, the generic and branded products contain the same active ingredient, meaning UltraGreen.ai is effectively offering the same product at two price points. This allows the group to retain more sales volume within its portfolio, but shifts part of that volume to a lower-priced product, potentially reducing its blended average selling price (ASP). A peer-reviewed study of FDA Competitive Generic Therapy approvals found that generic entry reduced the median price of affected drugs by around 18%. This provides a useful benchmark for potential price erosion, although the impact may differ for a specialist surgical injectable with an established incumbent.
New growth avenues remain at an early stage
Management indicated that Europe remains the fastest-growing region outside the US. Orders at a major Swiss hospital doubled within two months of clinical training as the hospital transitioned from technetium-99m, a radioactive tracer used in lymph node mapping, indicating the potential for wider ICG adoption in Europe. The approval of Verdye in South Africa has also expanded the group’s regulatory footprint to 42 countries. However, management acknowledged that international growth alone cannot fully offset the impact of the US business in the near term.
Beyond ICG, UltraGreen.ai is expanding its fluorescence imaging platform into new applications. The group is rolling out its IC-Flow imaging systems across the US, Europe and Asia, while extending portable imaging beyond the operating room into outpatient care and wound management, including diabetic foot ulcer clinics. PerfusionWorks, its perfusion analysis software, has also received European CE certification. These initiatives remain at an early stage, with the software still undergoing market testing and yet to contribute revenue. Management expects the imaging cameras to generate revenue first. While these businesses offer potential avenues for medium-term diversification, their contribution remains limited for now, leaving the group reliant on its core ICG franchise as it navigates increasing competition in the US.
Valuation
Taken together, the 3Q26 update confirms that the competitive risks highlighted in our previous update are now translating into a more challenging operating environment. The entry of Zydus and Provepharm, the relaunch of UltraGreen.ai’s own generic and management’s warning of continued pressure on US revenue and profitability in FY27 all point to a weaker earnings outlook. While the group’s established customer relationships and growing international business provide some support, its pricing power in the US can no longer be taken for granted. The ability to defend its market position will increasingly depend on how effectively it manages pricing and develops its newer businesses.
We downgrade UltraGreen.ai from HOLD to TRIM, while maintaining our fair P/E at 10x. Applying this multiple to our downwardly revised 2028E EPS yields a target price of USD 0.66. The lower target reflects a weaker earnings outlook, with the 3Q26 update confirming that competition has arrived earlier and from more directions than we assumed in our previous assessment. Against the closing price of USD 0.565 on 28 October, the target implies around 16.8% upside. However, we do not consider this potential upside sufficient to compensate for the heightened risks to earnings and market share as competition intensifies.
The key uncertainty remains the extent and pace of US price erosion, as well as the market share that Zydus and Provepharm can capture. With roughly three-quarters of revenue generated in the US, any shortfall in the group’s core market would have an outsized impact on earnings. Meanwhile, the imaging, software and outpatient care initiatives remain too early to offset the pressure on the core business. This combination makes near-term earnings increasingly difficult to forecast. At the current level of uncertainty, the potential upside is insufficient to justify maintaining a full position.
We would revisit our rating if there is meaningful evidence that the group’s strategy is effective, including stable US volumes at the new price level, early commercial traction from its imaging platform and outpatient applications, or greater clarity on the market share captured by Zydus and Provepharm. Until then, we advise reducing exposure on strength.
